Why do I need to provide Authorization Letters to submit my USA Passport application?

An authorization letter is a letter that authorizes Peninsula Visa to act as your representative and to submit your USA passport application. The United States Passport Agency requires that you provide this signed letter if you are not submitting your own application

  • the bottom portion of the letters must be entirely handwritten in blue ink only
  • check the first two boxes
  • write the applicant's name as in the passport. Last name first
  • the "Courier Company Name" is the name of the company on top of the form 
  • sign the application; the signatures should match the signature on the application form
  • all letters must be original with original signatures
